TRAUMA TREATMENT TEAM PROTOCOL
The
Sameem Associates, Inc. Trauma Treatment Team founded in 1998
to provide comprehensive out-patient evaluation and treatment
to adults, adolescents, children and families with histories of
trauma. This population includes clients who may have experienced
sexual or physical abuse, witnessed acts of violence, or victims
of natural disasters. Sameem Associates, Inc. TTT provides individual
and group psychotherapy, psychological testing, medication consultation
as well as drug, HIV and pregnancy testing. Our multidisciplinary
team of licensed and certified Professionals have specialized
training in both trauma treatment and in the treatment of addictive
behaviors including eating disorders.
Sameem Associates, Inc. also provides consultation to clinics,
hospitals, companies, schools and courts. Our current list of
clients are located across the United States, in the Caribbean
and Europe. Corporate clients have included the National Football
League, Legal Seafood, Hunt Petroleum, Elton-George, Kobacher
Co, Micro-Biomedics, Newton Public Schools, the Boston, Police
Dept., Gillette, etc.
Group treatment is a major part of our work at Sameem Associates,
Inc. We offer between 10-15 groups weekly that include recovery
groups for survivors both sexual abuse and violent traumas, adult
children of alcoholics groups, court referred offenders, groups
for affected family members, eating disorders groups, relapse
and recovery groups for men, women and adolescents.
Treatment Model
All counseling services begin with a telephone intake which documents
initial informational data on the identified patient and/or family
as well as the presenting problem. This is followed by a thorough
assessment to evaluate the needs of the individual and/or family.
Due to the training of our clinicians as well as our psychiatric
back-up we are able to effectively assess and treat for dual-diagnosis.
If indicated forensic cases are usually referred to other providers
for treatment upon completion of the evaluation, unless specified
by the court. In cases where there is an allegation of abuse of
one family member by another, each parent is assigned to a separate
therapist, and the alleged offender will have a separate therapist
from the alleged victim. This is to protect all family members.
In cases where there is an allegation of sexual or physical abuse
in the context of a child custody or visitation dispute, all family
members will be assigned separate clinicians (siblings may share
a clinician). Again, this is to protect the individual interests
of each family member as well as provide effective treatment.
Conjunct family sessions are only provided following the completion
of the initial evaluation and are not always indicated.
The Sameem Associates, Inc. TTT uses a team approach. Active cases
are conferenced at least once weekly in a meeting that all team
members are required to attend. This facilitates communication
between all clinicians involved in various cases and enables the
team to approach each case comprehensively.
Sameem Associates, Inc. TTT does not provide video or audio tapes
for forensic purposes nor do we provide polygraph tests. The team
members all have strong community networks and can make referrals
as needed.

Client Records
In Service Training
All new staff are oriented on record keeping protocol by the clinical
or executive director during their initial training. The clinical
supervisor again reviews the clinician's record keeping responsibilities
during regular supervision hours.
Protocol of Record Keeping
All Sameem Associates, Inc. records are given separate numbers.
Numbers and names are cross referenced and kept in a locked file
in the office. Records are not to be removed from the premises
and it is the responsibility of the primary clinician to insure
all appropriate and updated information is contained in the records.
This may include: urinalysis results, psychiatric reports from
hospitalization, correspondence with other treating agencies,
medical reports, progress notes, treatment plans, utilization
review, and diagnostic evaluations.
Clinicians must document each treatment session or any other contact
with the client on the `process notes' form and such must be done
on timely basis.
Diagnostic summaries must be completed after not more than four
sessions with the client.
Upon termination with the client, clinicians will complete a termination
summary to be included in the record.
Closed records are kept on the premises also in a locked file.
Staff Authorized to Make Entries
All clinical staff directly in contact are authorized to make
entries. In addition, the clinical supervisor and program director
are authorized to sign in their capacity as supervisor of UR review.
Consulting psychologists and psychiatrists are authorized to make
entries.
Access to Clinical Records
The clinician, the supervisor and the program director are authorized
to obtain access to records. Clinicians on call and clinician
covering for an absent primary therapist may obtain access to
records for a specific time period only.
