The Sameem Way

Seasons change and so do our lives, circumstances, and need for support.

 

Whether you are in crisis or working on your bucket list, we are here for you.

Sameem meets you where you are


We can support you in times of relative calm and mobilize quickly in the event of a crisis. 


Likewise, as people stabilize, we have less intense programming ready to help maintain progress and well-being.


All our programs are designed to help patients feel

  • optimistic about their futures
  • grateful for the lives they have today
  • connected to their closest friends or family and 
  • able to achieve goals they set for themselves. 


To reach these goals, Sameem has three levels of individualized support:

  • Long-term Healthy Living
  • Enchanted Support
  • Intensive Programs

Long-term Healthy Living

When the path is smooth, progress is easy.

Staying on track doesn’t always require a major time commitment. 


People at our Healthy Living level are not in crisis, but want to insure that they continue the progress they have made to healthy, connected, sober lives. They want to move forward -  to understand themselves better,  heal old wounds, strengthen their relationships, and find meaning in their lives. They participate in one or two Sameem sessions per week. 


Some people enter at this level of support and stay there; others transition from a higher level of support when that is called for.


TREATMENTS

A Healthy Living program can take many forms. Here are some common schedules:

  • Individual therapy only
  • A therapeutic group weekly and individual therapy twice a month
  • Two therapeutic groups and MAT services
  • Family counseling and a Harm Reduction group



TREATMENTS


Individual paths are again customized to what works best for them. Treatment continues to be less frequent as part of healthy living and their day to day is unaffected.  The type of support one may use is:


  • Individual therapy
  • A weekly therapeutic group meeting upon entering this phase
  • Family counseling
  • Medication management



HEALTHY LIVING GOALS & ACHIEVEMENTS

Whatever their goals, most people in Healthy Living grow in the these areas:


  • Increase in self understanding and ability to regulate emotions and behavior
  • Stronger connections to friends and family 
  • Optimism about the future and gratitude for the present
  • Ability to achieve goals they set for themselves
  • Reduced chance of serious relapse for those with substance use disorder


Enhanced Support

When the path is uphill, you need an extra hand

Maybe you’ve been on a smooth path but have encountered a job loss, sick or death of a loved one, or are facing a life changing event such as going off to college or a career change.  Or you have just completed an Intensive outpatient or inpatient program.


When a higher level of support is needed to ensure you get on the path and stay on the path to Healthier Living, individuals benefit from an enhanced support programs. The Enhanced Support includes three to five services per week, as needed. Programs are always individualized to meet individual needs.

 

Here are some examples of what Enhanced Support might look like:

  • Individual therapy, couples therapy, and a therapeutic group.
  • Family therapy, two or three therapeutic groups, and medically-assisted treatment (MAT).
  • Four therapeutic groups, plus individual therapy and MAT.

Intensive Support

If you fall, we’ll help you up

Intensive Outpatient Programs (IOPs) are typically for people coming out of medical detox or a residential program, or for those hoping to avoid having to take a step as drastic as inpatient care.   IOPs provide a rigorous solution to changing and improving your life. 


Clients in our IOPs receive 10 or more hours of services each week to help them get back on track. As always, we customize programs to meet your needs. If your goal is to continue to work or attend school, we can usually schedule to accommodate this. 

Initial Evaluation

& Ongoing Check-ins

Regardless of the level of care you seek at Sameem, your first appointment will be an evaluation with a therapist where you will co-create a care plan to meet your needs. Your Sameem care plan can typically start within 24-hours of this evaluation if needed.  Programs are customized based on an individual's need and their circumstances.


We usually suggest that patients make a three-month commitment, especially if they are entering following a setback of some kind. Even if an IOP runs only the first month, we recommend stepping down to Enhanced Support or to Long-term Healthy Living, because in general, making changes after a setback, or settling into new ways of thinking and acting, take a bit of time to secure the benefits.


The initial three months also serve as a more complete evaluation. At the three month mark, patients and their therapists make a longer term plan. For some people, this meeting is a formality - a quick check-in. 


For others, it’s a chance to review current well-being and decide whether progress or evolving needs should lead to any adjustments to services or level of program. 


Clinical measures, including the ASAM Criteria, Brief Addiction Monitor (BAM), Depression, Anxiety, and Stress Scale (DASS), ensure continuous assessment throughout the patient's tenure.
